Transportation Permission Form

I give permission for my child to ride the Faith Baptist Church bus/van. My child will be transported to the church and will be brought back to the same location that the child got on the bus/van. My child is also expected to sit in whatever seat he/she is instructed to sit in and to show kindness to others on the bus/van. Failure to show kindness or to follow instructions will result in the child not being allowed to ride the bus/van. Any communication—such as if you will be picking your child up at the church or if you want them let off of the bus/van with a friend who also rides the bus/van MUST be done by a written & signed note given to the driver or rider.

This information will be used to contact you in an emergency or any other special need that arises. We will also use this information to inform you of any special events at the Faith Baptist Church.
